Introduction To Principles of Care in Practice: A Case Study

In Health and social care, Professionalism is considered as an important aspect while providing care and medication support to an individual with specific needs. Professionalism ensures that the healthcare workers or the professionals are responsible and accountable enough to make sound decisions and actions to provide the best-fit treatment to the patient by maintaining competencies during their careers (Fitzgerald, 2020). Therefore, Professionalism will be patient-centered, and with professionalism, healthcare workers can promote the best possible outcome in care service by minimising the exposure to the risk of harm. The aim of this report is to apply three principles and values of Professionalism and highlight how it support the delivery of high-quality, safe, and compassionate care to Hassan. For this proposed report, a highlight has been given on Hassa who is a 48-year-old taxi driver, diagnosed with Type 2 diabetes, and various other health and social challenges with his family. The family of Hassan lived in a deprived area, close to the city center of Bradford. Due to family problems, and responsibilities, Hassa is struggling to manage his health and his GP refers to “me” as a social prescribing link worker to support him.

In health and social care practice, values refer to the attitude and beliefs that can guide the healthcare worker to showcase professional behaviour toward the patient. The value, according to Varkey, (2021), is considered as the foundation upon which health and social care service is based on and helps to promote compassionate, accountable, and responsible care service facilities to the individuals by maintaining their diversity, and dignity. On the other hand, according to Liebler & McConnell, (2020), principles can be defined as guidelines that can govern healthcare professionals to follow proper and standard behavioral approaches while dealing with patients. In the case of NHS, the principles for healthcare professionals are responsive care, support, respect, compassion, and dignity. These principles can help health and social care workers at NHS to promote a culture of care that is patient-centric, respectful, and inclusive, and can meet the unique needs and wishes of individuals with specific needs (Shanafelt et al. 2021). Considering the concept of “values” and “principles” in health and social care setup, it can be stated that professionals like social prescribing link workers must navigate the fundamental issues while working with service users, which is rooted in being following ethical practices, adherence to the code of conducts, and being accountable (Flessa & Huebner, 2021). The role of a health care worker in working with service users includes the promotion and use of effective communication skills, maintaining the privacy and dignity of the patient, and upholding the 6C's principles- care, compassion, competencies, communication, courage, and commitment towards duty (Goh et al. 2020).

In any health and social care work, the roles and responsibilities of a worker will be multi-faceted and will ensure that the service they are providing to the service users is patient-centric (Shanafelt et al. 2021). Considering the case scenario of Hassan, the role of a prescribing social link worker will be to address the biophysical needs of Hassam and encourage him to adopt a healthy lifestyle with the administration of proper medication through holistic care strategies. When the roles and responsibilities of a health care worker are to develop a tailored care plan and connect the service users with community resources, the prescribing social link worker should adhere to a strict code of conduct, and code of practices and will be accountable for following ethical practice while dealing with service users.

Guided by the 6C's principle- care, competence, compassion, communication, commitment, and courage, the care worker can ensure that service users like Hassan are eligible for getting person-centered care in both safe and compassionate ways based on his diverse needs.

Applying Person-Centred Care Standards to Support Hassan's Health and Wellbeing

From the case study, the focus has to be given to Hassan, who is a 48-year-old taxi driver, who lives with his wife Salima, and 4 children in a terraced house that is close to the city center of Bradford. According to the case scenario, Hassan has been diagnosed with type 2 diabetes, and he has high blood glucose levels due to the consumption of high-fat and carbohydrate-rich foods. Additionally, Hassan is a heavy smoker, and a lack of physical activity has led Hassan to develop chronic obstructive pulmonary disorder. Additionally, as per the case scenario, Hassan is grappling to manage his health and familial complexities. As a social prescribing link worker, the care standards that can be best -fit to provide standardised care to Hassan are:

1. Person-Centered Care
2. Effective Communication
3. Equality and diversity

Considering the case scenario of Hassan, the Person-Centred Care approach (Care Certificate Standard 5) will involve the tailoring of intervention in alignment with the biophysical, cultural, and personal preferences of the service users. According to (), Person-Centered Care in the healthcare approach refers to the practice of being responsive to the service users, their values, preferences, and needs. Mullin et al. (2021) have mentioned that, in the healthcare sector, the benefit of the Person-Centered Care standard is it can support and encourage the service users to develop the skill, knowledge, and confidence that they need to become efficient and able to manage and make informed decisions about their health and healthcare. With the Person-Centered Care approach, as a social prescribing link worker, one can encourage Hassan to get engaged in the shared decision-making and planning process of culturally viable dietary guidelines, by acknowledging his religious beliefs, and constraints. Another justification for choosing this standard is, this specific standard includes the integrated care approach, which fosters the coordination of healthcare workers to better identify and address the physical, behavioral, and mental needs of individuals like Hasan. More specifically, with the integrated care approach as a part of Person-Centered Care, the social prescribing link worker can set the achievable health goal for Hasan by designing the diet plan, and daily -life routine by considering his religion, belief, family, and economic scenario. For example, with the Person-Centred Care Approach, the Social Prescribing Link workers can suggest healthier alternatives within their cultural cuisine and make diet plans by taking rituals like fasting during Ramadan month into consideration (Islam, Muhamad & Leong, 2023).

The Role of Effective Communication and Equality and Diversity Standards

Considering the case scenario and addressing the lack of understanding of Hasan about managing blood glucose levels, diabetes, and other health-related issues, effective communication (Care Certificate Standard 6) will be the best-fit standard of care. By using clear, and accessible language, along with visual aids, it can be easy for social prescribing link workers to involve Hasan in the discussion of his current mental and physical health condition and empower him to actively take part in care strategies (Fitzgerald, 2020). Additionally, Effective communication can help service users like Hasan to be better educated about their health, which can make them able to embrace proactive health and wellbeing strategies (Mullin et al. 2021). Additionally, respectful communication between the social prescribing link worker and Hasan can reduce the uncertainty, and enhance better patient engagement in decision making which can help Hasan to better adhere to the medication and treatment plan. Above all with effective communication, it can be ensured that Hasan is getting consistent and informed care to reduce the risk of type 2 diabetes, high fat, and carbohydrate levels in the blood, along with the risk of chronic pulmonary obstructive disorder to a considerable extent (Flessa & Huebner, 2021). Through effective communication, the healthcare worker can also make Hasan aware of the benefits of physical activities and quitting smoking habits for his health and well-being in the long run.

Equality and Diversity are another important Care Certificate standards with which high-quality, effective, and safe care services can be provided to Hasan. The Care Certificate Standard 4, Equality and Diversity emphasises the importance of treating every individual with respect, dignity, and fairness, and value them by promoting diversity and providing inclusive care services that meet their wishes, needs, and requirements (Shanafelt et al. 2021) Considering the case scenario of Hasan, it can be stated that addressing his economic and familial scenario, the onset of the risk of Type 2 diabetes along with other healthcare challenges, the patient needs personalised care which can respect his religious belief, socioeconomic context, and health needs. By making accommodations for the Halal food, coordinating with the pharmacy department to ensure that there is an alternative to the medicine that uses gelatin or alcohol in their formulation, and making plans for physical activities by accommodating the time of daily prayer which is practiced five times in a day is the best possible way to maintain the equality and diversity of Hasan while offering him high-quality, safe, and effective healthcare services (Islam, Muhamad & Leong, 2023).

To conclude this report, it can be stated that professionalism is the pivotal part of any health and social care, which can ensure the responsibility, accountability, and commitment among healthcare professionals and their ability to make sound decisions for effective care plans for patients. This report has focused on applying three principles and values of professionalism to Hasan, who is a 48-year-old taxi driver facing health and familial challenges. In this report, three Care Certificate standards- Equality and Diversity, Effective Communication, and Person-Centered Approach provide a high-quality, safe, and effective care plan to Hasan based on his needs, and preferences. In a broader context, these three principles and values underscore the importance of adherence to the code of practices, code of conduct, ethical practices, and accountability while offering care to patients like Hasan. These care certificate standards contribute to the development patient-centric, effective, culturally viable, and inclusive care approach, by using which the social prescribing link worker can ensure positive health outcomes for the service users.
