Introduction To Principles of Care in Practice: A Case Study

In Health and social care, Professionalism is considered as an important aspect while providing care and medication support to an individual with specific needs. Professionalism ensures that the healthcare workers or the professionals are responsible and accountable enough to make sound decisions and actions to provide the best-fit treatment to the patient by maintaining competencies during their careers (Fitzgerald, 2020). Therefore, Professionalism will be patient-centered, and with professionalism, healthcare workers can promote the best possible outcome in care service by minimising the exposure to the risk of harm. The aim of this report is to apply three principles and values of Professionalism and highlight how it support the delivery of high-quality, safe, and compassionate care to Hassan. For this proposed report, a highlight has been given on Hassa who is a 48-year-old taxi driver, diagnosed with Type 2 diabetes, and various other health and social challenges with his family. The family of Hassan lived in a deprived area, close to the city center of Bradford. Due to family problems, and responsibilities, Hassa is struggling to manage his health and his GP refers to “me” as a social prescribing link worker to support him.

In health and social care practice, values refer to the attitude and beliefs that can guide the healthcare worker to showcase professional behaviour toward the patient. The Role of Effective Communication and Equality and Diversity Standards

Considering the case scenario and addressing the lack of understanding of Hasan about managing blood glucose levels, diabetes, and other health-related issues, effective communication (Care Certificate Standard 6) will be the best-fit standard of care. By using clear, and accessible language, along with visual aids, it can be easy for social prescribing link workers to involve Hasan in the discussion of his current mental and physical health condition and empower him to actively take part in care strategies (Fitzgerald, 2020). Additionally, Effective communication can help service users like Hasan to be better educated about their health, which can make them able to embrace proactive health and wellbeing strategies (Mullin et al. 2021). Additionally, respectful communication between the social prescribing link worker and Hasan can reduce the uncertainty, and enhance better patient engagement in decision making which can help Hasan to better adhere to the medication and treatment plan. By making accommodations for the Halal food, coordinating with the pharmacy department to ensure that there is an alternative to the medicine that uses gelatin or alcohol in their formulation, and making plans for physical activities by accommodating the time of daily prayer which is practiced five times in a day is the best possible way to maintain the equality and diversity of Hasan while offering him high-quality, safe, and effective healthcare services (Islam, Muhamad & Leong, 2023).

To conclude this report, it can be stated that professionalism is the pivotal part of any health and social care, which can ensure the responsibility, accountability, and commitment among healthcare professionals and their ability to make sound decisions for effective care plans for patients. This report has focused on applying three principles and values of professionalism to Hasan, who is a 48-year-old taxi driver facing health and familial challenges. In this report, three Care Certificate standards- Equality and Diversity, Effective Communication, and Person-Centered Approach provide a high-quality, safe, and effective care plan to Hasan based on his needs, and preferences. In a broader context, these three principles and values underscore the importance of adherence to the code of practices, code of conduct, ethical practices, and accountability while offering care to patients like Hasan. These care certificate standards contribute to the development patient-centric, effective, culturally viable, and inclusive care approach, by using which the social prescribing link worker can ensure positive health outcomes for the service users.
